I am an in-network provider with the following insurance plans: Aetna, BCBS PPO (excluding local and HPO plans), the State Health Plan, Cigna/Evernorth, Optum/United Behavioral Health, and Quest Behavioral Health.

Good Faith Estimate

In compliance with the No Surprises Act, this law requires us to provide you with a Good Faith Estimate of the total cost of your treatment. This applies for self-pay clients, that are not interested in pursuing out-of-network reimbursement from their insurance providers. The Good Faith Estimate providers clients with fee expectations while receiving therapeutic services. If a fee becomes associated with a service, the client will have knowledge of the fee and be required to accept responsibility for the charges prior to the services being provided.

Superbill for Reimbursement

​

If I am not in-network with your insurance carrier and you would like to work with me, I can provide you with a superbill (insurance invoice). This requires you to pay for the session at the time of your appointment. You will submit the superbill directly to your insurance provider to request reimbursement for services from them.
